- Just print the application below (page 2) and you can order several plates on one form.

- Fill out the application – Plates are $25 each.

- Write out your check payable to Kentucky State Treasurer

- Mail to Nancy Short, KWHA Ladies Auxiliary, P O Box 568, Richmond KY 40476-0568

- We must have 900 PAID applications before the plate will be produced.

- Your check will not be cashed until we get 900 applications, and only then will it be sent to Frankfort and they will cash it.

- The sooner we get 900 paid applications, the sooner we’ll get our plate. We must get our 900 applications by September 1 or the plate will not be produced. So HURRY! HURRY! HURRY! Send in your application and check without delay!

- Once we have 900 applications, you will be notified to pick up your plate(s) at your county clerk’s office. There’s a $3 charge by county clerk, but you’ve already paid for the plate. (Renewal is $15 yearly after 1^{st year.)}

- Farm tag – is not available at this time. You can put the plate on cars and trucks but not Farm tag vehicles. (After we have the plate for one year we can apply for farm tag approval.)

- If you sell or trade your car, you can take the plate with you.
